Thank you for selecting Wooster Community Hospital in Wooster, Ohio, for your medical care. If you are scheduled for a test or treatment at WCH, we will attempt to reach you by phone for preregistration. If we do not reach you, please feel free to call us.

At the time of preregistration, you will be given any instruction necessary for your tests to be completed and the location of the testing. Laboratory and some X-ray services are also available at the hospital's Milltown Service Center at 128 East Milltown Road in Wooster.

To help speed up the registration process, you may download, print, and complete the forms below prior to your visit.

Established patients can update forms online through our patient portal. If you need help accessing the portal, please call us and we’ll be happy to help.
